Application and Course Requirements

1. Application with official school seal and original signatures

Stamped signatures, faxes and photo copies can not be accepted.

To complete an application please go to the following UCLA website:

www.medstudent.ucla.edu/visitingstudents/

2. Medical School transcripts with the official school seal

Faxes and photo copies can not be accepted.

 

3. Documentation of a passing grade in core medicine and surgery rotations

There must be a final passing grade documented on your official transcript for these courses or your application must include an official letter from the medicine or surgery course chairman stating that you have passed the course. Pending grades or a letter stating that you will pass the course can not be accepted.

4. $100 application fee to "Regents of the University of California"

Note: Our department does not charge a fee. This is a University of California requirement.

5. Proof of personal health and accident insurance

Please provide proof of school or personal insurance that covers you during your rotation.

6. Risk management fee if not covered by your medical school

Please provide proof of risk management coverage by your institution during your rotation.

7. Letter of Recommendation from a medical school faculty member

Please provide a letter or recommendation from a clinical faculty member who has worked with you. The "Dean's Letter" will not suffice for this requirement.

8. UCLA Health evaluation form filled out and signed by a physician

9. Photograph with your signature on the back attached to the UCLA photo form

10. Final approval from UCLA School of Medicine

All applications will be approved by Dr. Coates and UCLA approximately 1 month prior to the start of the rotation. Applications are approved on merit and not on a first-come, first-served basis. Unfortunately, all applicants can not be offered a rotation.
